Job Description

Deliver compassionate, high-quality care while enjoying the flexibility of per diem work as a Nurse Practitioner specializing in home health risk assessments. In this dynamic role, youll travel within Union County, ARincluding areas such as El Dorado, Smackover, and Norphletto perform comprehensive risk assessments for Medicare Advantage plan members.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to best suit your needs
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments in members residences
  • Review and evaluate medical history, current conditions, and medication regimens
  • Educate members on health conditions, medications, and general wellness
  • Identify and address gaps in care, providing actionable recommendations
  • No need to create treatment plans or prescribe medicationsfocus on thorough, quality assessments

Qualifications:

  • Active Nurse Practitioner License in Arkansas
  • Board Certification (AANP or ANCC) required
  • Current CPR certification (BLS)
  • Completion of an MSN degree from an accredited nursing school
  • Minimum 6 months of recent NP experience preferred
