Advance your career as a Nurse Practitioner in a dynamic home health setting, conducting comprehensive risk assessments for Medicare Advantage plan members across Cedar County, MO. This per diem opportunity offers significant flexibility, allowing you to tailor your schedule and work in diverse communities such as Stockton, El Dorado Springs, and Jerico Springs.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your personal and professional commitments
- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform annual risk assessments for members in a home environment
- Review prior and current medical history, conditions, and medication use
- Educate members about their health conditions and medications
- Identify care gaps and provide insightful recommendations for improvement
- No prescribing or treatment plan development required; primary focus is on assessment and education
Qualifications:
- Active Nurse Practitioner license in Missouri
- Current Basic Life Support (BLS/CPR) certification
- Board certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of current Nurse Practitioner experience preferred
