Job Description
Make a real impact in patients lives by providing home health risk assessments on a flexible, per diem basis. As a Nurse Practitioner, youll use your clinical expertise to evaluate Medicare Advantage plan members throughout Nodaway County, MOincluding Maryville, Burlington Junction, and Hopkinswhile enjoying the freedom to design your own schedule.
Schedule/Hours:
- Flexible shifts available every day of the week (Monday through Sunday)
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct individualized annual health assessments and post-discharge visits in patients homes
- Perform comprehensive reviews of medical histories, current conditions, and medication regimens
- Educate patients on their medical conditions and prescriptions, fostering improved health understanding
- Identify gaps in patient care and recommend interventions, ensuring thorough care follow-ups
- No responsibility for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Missouri
- Board Certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited school of nursing
- At least 6 months of recent Nurse Practitioner experience (preferred)
