Make a meaningful impact in the community by providing in-home health risk assessments for Medicare Advantage plan members. This per diem opportunity offers Nurse Practitioners the flexibility to create their own schedules and serve individuals across Cedar County, including El Dorado Springs, Stockton, and Jerico Springs. Each visit typically lasts around 45 minutes and may include annual health assessments or post-discharge visits, helping to ensure optimal outcomes for patients in the comfort of their own homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Option to design your own schedule
- Set your preferred weekly or bi-weekly hourly commitment
Qualifications:
- Active Nurse Practitioner license in Missouri
- CPR certification (BLS) required
- Board Certification through AANP or ANCC
- A minimum of six months current experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) degree from an accredited nursing program
Key Responsibilities:
- Conduct comprehensive, in-home annual health risk assessments for Medicare Advantage members
- Review, evaluate, and document prior and current medical histories, existing conditions, and medication lists
- Provide education to members regarding their health status, conditions, and prescribed medications
- Identify gaps in care and recommend appropriate steps for resolution
- No requirement to prescribe medication or create treatment plans
