This role involves conducting comprehensive home health risk assessments for Medicare Advantage plan members in Shawano County, WI, including locations such as Shawano, Bonduel, and Birnamwood. Schedule:

• Flexible shifts, Monday through Sunday
• Ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members
• Review and document prior and current medical history, medical conditions, and medications
• Educate patients on their health conditions and prescribed medications
• Identify gaps in care and offer actionable recommendations to support optimal patient health
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Wisconsin
• Board Certification via AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited program
• Current CPR certification (BLS)
• Minimum of six months recent NP experience preferred
