Youll engage patients in their homes in areas such as Hinesville, Midway, and Riceboro, conducting thorough evaluations that support care continuity and preventive health.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Commitment to minimum weekly or bi-weekly hours
Key Responsibilities:
- Perform comprehensive annual risk assessments and post-discharge visits for Medicare Advantage members
- Evaluate prior and current medical history, existing medical conditions, and medication use
- Educate patients about chronic conditions, healthy living, and safe medication practices
- Identify and document gaps in care, offering recommendations for resolution
- No treatment plan development or medication prescribing required
Qualifications:
- Current Georgia Nurse Practitioner license
- Active CPR certification (BLS)
- National Board Certification (AANP or ANCC)
- At least 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited program
