Job Description

Deliver vital care in the community as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members throughout Chatham County. This per diem opportunity allows you to make a real impact by visiting patients in the comfort of their homesincluding locations in Savannah, Pooler, and Port Wentworth. You'll conduct thorough annual health evaluations or post-discharge reviews, typically spending about 45 minutes with each member.

  • Flexible shifts available between Monday and Sunday

Essential Responsibilities:

  • Complete comprehensive home health risk assessments
  • Review and evaluate patients prior/current medical histories, conditions, and medication use
  • Educate patients on their diagnoses and prescribed medications
  • Identify care gaps and make recommendations for follow-up and resolution
  • There is no requirement to create treatment plans or prescribe medications

Required Qualifications:

  • Active Nurse Practitioner license in the state of Georgia
  • Board Certification from AANP or ANCC
  • MSN from an accredited nursing program
  • Current CPR (BLS) certification
  • At least 6 months of recent Nurse Practitioner experience is preferred
