Job Description
Deliver vital care in the community as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members throughout Chatham County. This per diem opportunity allows you to make a real impact by visiting patients in the comfort of their homesincluding locations in Savannah, Pooler, and Port Wentworth. You'll conduct thorough annual health evaluations or post-discharge reviews, typically spending about 45 minutes with each member.

Essential Responsibilities:
- Complete comprehensive home health risk assessments
- Review and evaluate patients prior/current medical histories, conditions, and medication use
- Educate patients on their diagnoses and prescribed medications
- Identify care gaps and make recommendations for follow-up and resolution
- There is no requirement to create treatment plans or prescribe medications
Required Qualifications:
- Active Nurse Practitioner license in the state of Georgia
- Board Certification from AANP or ANCC
- MSN from an accredited nursing program
- Current CPR (BLS) certification
- At least 6 months of recent Nurse Practitioner experience is preferred
