Key Responsibilities:

  • Perform annual and post-discharge home health risk assessments, typically lasting about 45 minutes per visit
  • Evaluate patients prior and current medical history, medical conditions, and medication usage
  • Educate patients on chronic conditions, medication regimens, and healthy practices
  • Identify healthcare gaps and offer clinical recommendations to address needs
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Georgia
  • MSN from an accredited nursing school
  • Board Certification as a Nurse Practitioner through AANP or ANCC
  • Valid CPR certification (BLS)
  • At least six months of recent Nurse Practitioner experience preferred
