Experienced Nurse Practitioners are sought for per diem home health risk assessment visits within Camden County, GA. Shape your own flexible schedule while making a difference in the lives of Medicare Advantage members throughout Kingsland, St. Marys, and Woodbine. As a vital contributor to preventative healthcare, youll provide valuable insight through annual and post-discharge health assessments in the comfort of the patients home.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Thoroughly review and evaluate members prior/current medical history, medications, and conditions
- Educate members on disease management, medication adherence, and overall health maintenance
- Identify gaps in care and offer recommendations to enhance overall patient outcomes
- No requirement for treatment plan creation or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Georgia (GA)
- Board Certification (AANP or ANCC)
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
