Youll conduct comprehensive, 45-minute home visitsfocusing on annual health or post-discharge assessments throughout Mount Olive, Wallace, Warsaw, and other areas within Duplin County.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own working schedule
• Meet minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform in-home health risk assessments for Medicare Advantage health plan members
• Review medical history, evaluate current conditions, and document medication use
• Educate patients on their conditions and medications to foster informed health decisions
• Identify gaps in care and make recommendations for further action
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification through AANP or ANCC
• MSN from an accredited nursing school
• Current CPR certification (BLS)
• At least six months of nurse practitioner experience preferred

As a valued part of this home health initiative, you will be compensated $80$90 per evaluation, along with a mileage stipend and additional pay for administrative time.
