Deliver essential home health care as a Nurse Practitioner, conducting comprehensive risk assessments for Medicare Advantage plan members across various communities in Northampton County, NCincluding Gaston, Garysburg, and Rich Square. This per diem opportunity offers flexible scheduling, ideal for professionals seeking autonomy over their work hours.

Schedule:

• Flexible shifts available Monday through Sunday
• Build your own work schedule
• Minimum weekly or bi-weekly hourly commitment as preferred

Responsibilities:

• Conduct detailed home health risk assessments, averaging 45 minutes per visit, for Medicare Advantage members
• Gather and review medical history, existing conditions, and medication usage
• Educate members on their health conditions and medication management
• Assess gaps in care and recommend appropriate next steps
• Visits may include annual health assessments or post-discharge follow-ups
• No responsibility for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Current CPR certification (BLS)
• Board Certification (AANP or ANCC)
• MSN from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred
