Job Description

Step into a vital role supporting community health as a Nurse Practitioner specializing in home health risk assessments within Adair County and surrounding areas, including Kirksville, Greentop, and Novinger. This per diem position focuses on delivering high-quality care to Medicare Advantage plan members in their own homes.

Schedule and Commitment:

  • Flexible shifts offered any day between Monday and Sunday
  • Freedom to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive home health risk assessments during each patient visit, averaging approximately 45 minutes
  • Review and evaluate both prior and current medical and medication histories
  • Educate individuals on their medical conditions and medication use
  • Identify any gaps in care, making recommendations to address them
  • Visits may include annual health evaluations or post-discharge follow-ups
  • No need to develop treatment plans or prescribe medications

Qualifications:

  • Active and unrestricted Nurse Practitioner license in Missouri
  • Current CPR certification (BLS)
  • Board Certification via AANP or ANCC
  • Graduation from an accredited nursing program with an MSN degree
  • At least 6 months of recent NP experience is preferred
