Step into a vital role supporting community health as a Nurse Practitioner specializing in home health risk assessments within Adair County and surrounding areas, including Kirksville, Greentop, and Novinger. This per diem position focuses on delivering high-quality care to Medicare Advantage plan members in their own homes.
Schedule and Commitment:
- Flexible shifts offered any day between Monday and Sunday
- Freedom to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments during each patient visit, averaging approximately 45 minutes
- Review and evaluate both prior and current medical and medication histories
- Educate individuals on their medical conditions and medication use
- Identify any gaps in care, making recommendations to address them
- Visits may include annual health evaluations or post-discharge follow-ups
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active and unrestricted Nurse Practitioner license in Missouri
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- Graduation from an accredited nursing program with an MSN degree
- At least 6 months of recent NP experience is preferred
In return for your expertise, you will be compensated $80-$90 per evaluation, with additional mileage stipends and administrative time included.
