Job Description

Nurse Practitioners with experience in home health are needed to conduct risk assessments for Medicare Advantage plan members across Chatham County, GA. This per diem role offers the flexibility to create your own schedule, with shifts available seven days a week. Youll perform comprehensive assessments, averaging 45 minutes per visit, ranging from annual health reviews to post-discharge evaluations. Candidates may be assigned to various locations within the county, including Savannah, Pooler, and Port Wentworth, so a willingness to travel within the area is essential.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling; create your own calendar
  • Options for minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct detailed home health risk assessments for Medicare Advantage members
  • Review and evaluate prior/current medical histories, active conditions, and medication usage
  • Educate patients about their health conditions and proper medication management
  • Identify deficits or opportunities in care and provide practical recommendations
  • No requirement for prescribing medications or designing treatment plans

Qualifications:

  • Active Nurse Practitioner license in Georgia
  • Board Certification via AANP or ANCC
  • Current Basic Life Support (CPR/BLS) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of recent nurse practitioner experience preferred
