A fulfilling opportunity is available for Nurse Practitioners who are interested in per diem home health roles. This position focuses on conducting home health risk assessments for Medicare Advantage plan members residing throughout Wilson County, NC. These assessments, which typically take about 45 minutes, include annual health evaluations as well as post-discharge visits and may be scheduled in locations such as Wilson, Elm City, and Lucama within Wilson County.

Schedule:

• Flexible shifts, offered between Monday and Sunday
• Design your own work schedule
• Minimum weekly or bi-weekly hourly commitment according to your availability

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage plan members
• Review past and current medical history, diagnoses, and medication records
• Educate patients on their medical conditions and medications
• Identify care gaps and make recommendations for appropriate next steps
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Current BLS (CPR) certification
• Board Certification through AANP or ANCC is required
• At least 6 months of recent NP experience preferred
• Master of Science in Nursing (MSN) from an accredited nursing program

This role offers $80-$90 per completed evaluation, plus a mileage stipend for travel and administrative time.
