Advance your career as a Nurse Practitioner specializing in home health with a role focused on conducting home health risk assessments for Medicare Advantage plan members. This per diem opportunity allows you to serve patients in Peebles and surrounding areas of Adams County, with the flexibility to tailor your schedule to your needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate prior and current medical history, health conditions, and medication use
- Educate patients on their medical conditions and prescribed medications
- Identify and document any care gaps, offering clinical recommendations to resolve issues
- Visits typically last about 45 minutes each; annual assessments and post-discharge visits may be required
- No treatment plan creation or medication prescribing involved
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current BLS/CPR certification
- Board Certification with AANP or ANCC
- At least 6 months of experience practicing as a Nurse Practitioner preferred
- Graduation from an accredited MSN program
This role offers competitive pay, with $80-$90 earned per evaluation, in addition to mileage reimbursement and compensation for administrative time.
