This per diem opportunity allows you to serve patients in Peebles and surrounding areas of Adams County, with the flexibility to tailor your schedule to your needs.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage plan members
• Review and evaluate prior and current medical history, health conditions, and medication use
• Educate patients on their medical conditions and prescribed medications
• Identify and document any care gaps, offering clinical recommendations to resolve issues
• Visits typically last about 45 minutes each; annual assessments and post-discharge visits may be required
• No treatment plan creation or medication prescribing involved

Qualifications:

• Active Nurse Practitioner license in Ohio
• Current BLS/CPR certification
• Board Certification with AANP or ANCC
• At least 6 months of experience practicing as a Nurse Practitioner preferred
• Graduation from an accredited MSN program

This role offers competitive pay, with $80-$90 earned per evaluation, in addition to mileage reimbursement and compensation for administrative time.
