Experience the rewarding opportunity to deliver essential care to Medicare Advantage plan members across Stephenson County, Illinois. As a per diem Nurse Practitioner specializing in home health, you will conduct comprehensive risk assessments in patients residencesincluding communities such as Freeport, Lena, and Orangeville. Each visit typically spans about 45 minutes and may involve either annual assessments or post-discharge evaluations. Your skillful guidance will help ensure patients receive the support and education they need while identifying and addressing potential gaps in care.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingdesign your own workweek
- Choose a minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Complete annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Assess and document patients past and present medical history, including conditions and medication regimens
- Provide clear, compassionate education regarding diagnoses and prescribed medications
- Offer actionable recommendations and highlight any care gaps
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Illinois
- Board certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least six months of recent experience as a Nurse Practitioner (preferred)
