Experienced Nurse Practitioners are needed to perform home health risk assessments for Medicare Advantage members in communities throughout Kent County, RI, such as Warwick, West Warwick, and Coventry. This independent per diem opportunity offers the flexibility to tailor shifts to your lifestyle, with the option to create your own schedule and commit to minimum weekly or bi-weekly hoursideal for those who value work-life balance.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: create your own work schedule
• Minimum weekly or bi-weekly hour requirement

Responsibilities:

• Conduct comprehensive home health risk assessments during visits averaging 45 minutes each
• Evaluate patients current and prior medical history, including conditions and medications
• Provide clear, concise education regarding chronic conditions and medication regimens
• Identify care gaps and suggest actionable recommendations for follow-up
• No prescribing or treatment plan development required

Qualifications:

• Current Nurse Practitioner licensure in Rhode Island
• Active board certification through AANP or ANCC
• Valid CPR certification (BLS)
• MSN degree from an accredited nursing program
• At least 6 months of recent NP experience is preferred
• Strong communication and patient education skills
