Deliver high-impact healthcare in a per diem role, conducting comprehensive home health risk assessments for Medicare Advantage plan members within Payette County, ID. This position offers flexibility and autonomy, with opportunities to serve individuals throughout Payette, Fruitland, and New Plymouth. Tailor your work to your lifestyle by setting your own schedule, making a difference one assessment at a time.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-schedulingdesign shifts around your availability
- Choose from weekly or bi-weekly minimum hourly commitments
Responsibilities:
- Perform thorough home health risk assessments, typically lasting about 45 minutes
- Assess current and past medical history, existing health conditions, and medication regimens
- Educate patients about their medical conditions and prescribed medications
- Identify care gaps, providing actionable recommendations to optimize patient outcomes
- Focus exclusively on assessmentno prescribing or treatment plan development required
Qualifications:
- Current Nurse Practitioner license in Idaho
- Certification in Basic Life Support (CPR)
- Board certification via AANP or ANCC
- MSN degree from an accredited nursing program
- At least six months of recent Nurse Practitioner experience preferred
