This opportunity offers a rewarding role delivering comprehensive healthcare to insurance members living in senior communities and home settings throughout Oregon City, OR. The primary focus is to treat patients in place, minimizing unnecessary hospitalizations by providing thorough monthly health assessments and timely interventions. This role supports improved patient outcomes by offering preventive, urgent, and acute care directly in the home or senior living environment.
Key Responsibilities:
- Develop individualized problem lists and medical treatment plans for patients.
- Provide primary and urgent care onsite to meet patient needs.
- Identify and manage high-risk patients through more frequent clinical visits.
- Create contingency plans tailored to high-risk conditions.
- Collaborate closely with primary care physicians and facility staff including Directors of Nursing.
- Deliver hands-on bedside care, develop care plans, and place medical orders.
- Conduct single visits for discharged patients transitioning home from skilled nursing facilities.
Qualifications:
- Master of Science in Nursing (MSN) degree.
- Active Oregon Nurse Practitioner (NP) license.
- Basic Life Support (BLS) certification.
- DEA license to prescribe medications.
- Preferred experience in Home Health, Hospice, or similar settings.
Schedule:
- Monday through Friday, 8 AM to 5 PM.
- No weekends or holidays to maintain work-life balance.
