Job Description

This role focuses on providing critical healthcare evaluations for Medicare Advantage plan members throughout Erie County, including areas such as Sandusky, Huron, and Vermilion. Each assessment, lasting approximately 45 minutes, may cover annual health evaluations or post-discharge visits, helping to ensure continuity and quality of care for patients in their own homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your personal and professional commitments
  • Minimum weekly or bi-weekly hourly commitment as determined by your availability

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Gather and review detailed medical history, existing conditions, and medication usage
  • Educate patients on diagnosed medical issues and prescribed medications
  • Detect care gaps and recommend strategies for improved health outcomes
  • No requirement for developing treatment plans or prescribing medication

Qualifications:

  • Current Nurse Practitioner license in Ohio
  • Board certification via AANP or ANCC
  • Valid CPR (BLS) certification
  • Minimum of 6 months experience as an NP preferred
  • MSN from an accredited nursing school
