Deliver high-quality patient care as a Nurse Practitioner providing home health risk assessments on a per diem basis throughout Cedar County, Missouri. This role focuses on visiting Medicare Advantage members at home in areas including El Dorado Springs, Stockton, and Jerico Springs. Each evaluation typically takes about 45 minutes and may be an annual health assessment or a post-discharge visit. The position is ideal for professionals seeking schedule flexibility and an opportunity to make a tangible impact in home-based care.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-schedulingdetermine your own hours
• Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

• Perform comprehensive annual risk assessments for Medicare Advantage plan members at their homes
• Review prior and current medical history, chronic conditions, and medication use
• Educate patients on health conditions and proper medication management
• Identify care gaps and provide actionable guidance to help resolve them
• No treatment planning or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in Missouri
• Current CPR certification (BLS)
• Board certification through AANP or ANCC
• Minimum of 6 months experience as a Nurse Practitioner preferred
• Masters degree in Nursing (MSN) from an accredited program
