This per diem position offers nurse practitioners the flexibility to design their own schedules, delivering crucial services where they're needed mostincluding locations such as Sterling, Rock Falls, and Morrison.
Schedule:
- Flexible shifts available between Monday and Sunday
- Set your own workdays and hours
- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct thorough annual home health risk assessments to support a Medicare Advantage health plan
- Evaluate each members prior and current medical history, including chronic and acute medical conditions and medications
- Deliver clear education about medical conditions and medication regimens
- Identify gaps in care and make recommendations to support member health
- Focus on assessment and educationno requirements for developing treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner License in Illinois
- Board certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- Minimum of 6 months NP experience preferred
This unique home health assignment provides a compensation of $80-$90 per evaluation, including a mileage stipend and administrative time compensation.
