A Nurse Practitioner with home health experience is needed to perform risk assessments on Medicare Advantage plan members in Jackson County. The role focuses on completing home-based health evaluations in locations such as Medford, Ashland, Eagle Point, and Jacksonville. Assessments typically last around 45 minutes and may include annual wellness checks or post-discharge visits. Scheduling is highly flexible, offering per diem opportunities to accommodate your lifestyle.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: create your own shifts
• Minimum weekly or bi-weekly hourly commitment required

Primary Responsibilities:

• Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
• Review and evaluate medical histories, current medical conditions, and medications
• Educate patients about their medical conditions and proper use of medications
• Identify care gaps and recommend actions to address them
• No treatment plans or prescribing responsibilities

Qualifications:

• Active Nurse Practitioner license in Oregon
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• MSN from an accredited school of nursing
• At least 6 months of Nurse Practitioner experience preferred

This position offers $80$90 per evaluation, with mileage reimbursement and additional compensation for administrative time.
