Enjoy the flexibility of per diem work by providing home health services to Medicare Advantage members in communities throughout Nodaway County, MO. This dynamic nurse practitioner opportunity focuses on conducting home health risk assessments in patient homes, supporting a proactive approach to care, and helping optimize member health outcomes. Assessments generally require approximately 45 minutes per visit and may consist of annual wellness visits or post-discharge assessments.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete home health risk assessments on Medicare Advantage members residing throughout Nodaway County, covering communities such as Maryville, Burlington Junction, and Hopkins
- Evaluate and document current and previous medical history, chronic conditions, and medication usage
- Educate patients regarding their diagnoses, medications, and general health maintenance
- Identify care gaps and provide recommendations to address and resolve them
- No treatment planning, prescriptions, or ongoing management required
Qualifications:
- Active Missouri Nurse Practitioner license
- MSN degree from an accredited nursing school
- Board certification through the AANP or ANCC
- Current BLS (CPR) certification
- At least six months of recent experience working as an NP is preferred
