Make a real difference for seniors in your community by providing high-quality home health risk assessments on a flexible, per diem basis. As a Nurse Practitioner specializing in home health, you'll visit Medicare Advantage plan members across Daviess County, Kentuckyincluding areas such as Owensboro, Whitesville, and Maceoconducting in-home evaluations that support better health outcomes.
Schedule:
- Flexible shifts available any day of the week (MondaySunday)
- Choose your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Core Responsibilities:
- Perform comprehensive annual in-home health risk assessments for Medicare Advantage members
- Review patients medical and medication histories, as well as prior and current conditions
- Offer guidance and education to patients regarding their health conditions and prescriptions
- Detect care gaps and recommend solutions to address those needs
- No treatment planning or medication prescribing involved
- Each assessment typically takes about 45 minutes
Qualifications:
- Active Nurse Practitioner license in Kentucky
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- 6 months or more of recent nurse practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited nursing school
For each evaluation completed, compensation ranges from $80 to $90, with a mileage stipend and payment for administrative time included.
