Play a vital role in delivering high-quality home health care as a Nurse Practitioner conducting risk assessments for Medicare Advantage plan members across Dent County, MO. This per diem opportunity offers the flexibility to create your own schedule, with shifts available any day of the week. Youll visit members in their homes, including areas such as Salem and Bunker, providing comprehensive assessments that support accurate care planning and improved health outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your preferences
- Commitment can be set on a weekly or bi-weekly basis
Key Responsibilities:
- Complete annual home health risk assessments (approx. 45 minutes each)
- Evaluate prior/current medical history, health conditions, and medication histories
- Educate members about their health conditions and medication management
- Identify gaps in care and recommend solutions for resolution
- No treatment planning or medication prescribing involved
Qualifications:
- Active Nurse Practitioner license in MO
- Board certification via AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- Minimum of 6 months current Nurse Practitioner experience preferred
