Seeking a skilled Nurse Practitioner with experience in home health to conduct comprehensive risk assessments for Medicare Advantage plan members in Washington County, MS. This per diem opportunity offers the flexibility to create your own schedule with shifts available between Monday and Sunday, allowing you to tailor your hours around your lifestyle. Youll serve patients throughout the Greenville, Leland, and Hollandale areas, making a significant impact on their overall well-being and continuity of care.

Key Responsibilities:

• Complete annual home health risk assessments in patient residences, averaging 45 minutes each
• Review and evaluate patients prior and current medical history, ongoing medical conditions, and medication records
• Educate patients about their diagnoses, treatment, and prescribed medications
• Identify care gaps and provide actionable recommendations to enhance patient outcomes
• Focus on assessmentcreation of treatment plans and prescribing are not required

Schedule:

• Flexible workdays: Monday through Sunday
• Set your own shifts
• Minimum weekly or bi-weekly hourly commitment

Qualifications:

• Active Nurse Practitioner license in Mississippi
• MSN from an accredited nursing program
• Board certification (AANP or ANCC)
• Current CPR certification (BLS)
• Minimum six months of recent NP experience preferred
• Strong communication and patient education skills
