Job Description
Deliver quality patient-centered care as a Nurse Practitioner specializing in home health assessments across Calcasieu County, including Lake Charles, Sulphur, and DeQuincy. This per diem opportunity allows you to make a meaningful impact by providing in-home risk assessments for Medicare Advantage plan members.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct comprehensive annual risk assessments during home health visits
- Review and evaluate patients medical histories, current conditions, and medication regimens
- Educate patients regarding their health conditions and prescribed medications to promote better self-management
- Identify any gaps in care and recommend action steps to help resolve those gaps
- Focus visits on evaluation and educationno requirement for treatment planning or medication prescribing
Qualifications:
- Active Nurse Practitioner license in Louisiana
- Current CPR (BLS) certification
- Board Certification from AANP or ANCC
- At least 6 months of NP experience preferred
- Master of Science in Nursing (MSN) from an accredited nursing school
