Job Description
Deliver high-quality care and make a meaningful impact in your community through in-home health risk assessments. This per diem opportunity is designed for Nurse Practitioners skilled in home health, offering the flexibility to create your own schedule while working within Howell County, MOincluding West Plains, Mountain View, and Willow Springs. With each assessment lasting around 45 minutes, you will engage with Medicare Advantage plan members, performing either annual health assessments or post-discharge visits depending on patient needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling optionschoose your own working days and hours
- Minimum weekly or bi-weekly hourly commitment ensures adaptability
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Review and assess past and present medical history, ongoing conditions, and medication usage
- Educate patients about their health conditions and medications
- Identify gaps in care and provide recommendations to optimize patient outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Missouri
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- At least 6 months of recent NP experience preferred
- MSN degree from an accredited nursing program
This position offers a competitive rate of $80-$90 per completed evaluation, with additional compensation provided for mileage and administrative tasks.
