Deliver exceptional patient-centered care with a rewarding opportunity conducting home health risk assessments as a Nurse Practitioner. This per diem role focuses on visiting Medicare Advantage plan members in their homes within Adair County, including areas such as Kirksville, Greentop, and Novinger. Assessments, typically lasting 45 minutes each, may range from annual health screenings to post-discharge follow-ups, providing a vital link in the continuum of care.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: Create your own work calendar
- Minimum weekly or bi-weekly hour commitment
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage members
- Review prior and current medical and medication histories
- Educate patients on health conditions and prescribed medications
- Identify care gaps and suggest actionable recommendations
- No requirement for prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in Missouri
- Board Certification from AANP or ANCC
- Current BLS (CPR) certification
- MSN from an accredited nursing school
- At least six months of recent Nurse Practitioner experience preferred
