Job Description
Deliver exceptional patient care and autonomy as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage members. In this per diem position, youll visit individuals across Camden County, including Kingsland, St. Marys, and Woodbine, providing important health evaluations and education in the comfort of their homes.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Select a minimum hourly commitment on a weekly or bi-weekly basis
Role Overview:
- Perform comprehensive home health risk assessments (approx. 45 minutes per visit) for Medicare Advantage plan members
- Assess prior and current medical history, various medical conditions, and medication history
- Offer education to patients about their medical conditions and prescribed medications
- Identify potential gaps in care and recommend actionable next stepsno treatment planning or prescriptions required
- Visits may include different types such as annual wellness or post-discharge assessments
What We're Looking For:
- Active Nurse Practitioner License issued by the state of GA
- Board Certification (AANP or ANCC) required
- Current BLS/CPR certification
- MSN degree achieved from an accredited program
- At least 6 months of recent NP experience preferred
