Advance your clinical experience and make a meaningful impact by providing home health risk assessments within Camden County. This per diem role offers flexible scheduling and the freedom to create your own shifts, with opportunities available Monday through Sunday. Youll serve Medicare Advantage plan members in Kingsland, St. Marys, Woodbine, and surrounding areas, conducting vital assessments to ensure member well-being.
Schedule:
- Flexible shifts available any day of the week
- Design your own schedule to fit personal and professional commitments
- Minimum weekly or bi-weekly hourly requirements
Qualifications:
- Valid Nurse Practitioner license in Georgia
- Board Certification from AANP or ANCC
- MSN from an accredited school of nursing
- Current CPR certification (BLS)
- At least 6 months of recent NP experience preferred
Key Responsibilities:
- Complete comprehensive annual and post-discharge risk assessments for Medicare Advantage members
- Gather and review patient medical history, existing conditions, and medication use
- Educate patients about their health conditions and prescribed medications
- Detect care gaps and recommend possible resolutions
- No requirement for treatment plan creation or medication prescribing
