Deliver expert care and support to Medicare Advantage plan members by conducting crucial home health risk assessments. This per diem opportunity gives Nurse Practitioners the flexibility to create their own schedules, serving patients throughout Clarke County, including areas such as Athens and Winterville. The position focuses on thorough, one-on-one assessments in patients' homes, each averaging 45 minutes, and may include annual wellness visits or post-discharge evaluations.

Schedule:

• Flexible shifts available any day Monday through Sunday
• Choose and manage your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual risk assessments for Medicare Advantage members in home settings
• Review and document current and past medical history, ongoing medical conditions, and medication use
• Provide clear education to patients regarding their medical conditions and prescribed medications
• Assess for gaps in medical care and offer concrete recommendations to address them
• No requirement to create treatment plans or prescribe medications

Desired Qualifications:

• Active Nurse Practitioner License in the state of Georgia
• Current CPR certification (BLS)
• Certification through AANP or ANCC as a Nurse Practitioner
• At least six months of current NP experience preferred
• MSN degree from an accredited nursing program
