• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, conditions, and medication history during each 45-minute visit
• Provide patient education on medical conditions and medications
• Identify gaps in care and recommend follow-up actions
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Louisiana
• Current CPR/BLS certification
• Board Certification (AANP or ANCC)
• MSN degree from an accredited nursing school
• At least 6 months of current nurse practitioner experience preferred

You'll be seeing members in various locations within Calcasieu County, such as Lake Charles, Sulphur, and DeQuincy, providing crucial healthcare assessments both for annual wellness and post-discharge follow-ups.

Compensation for this role is highly competitiveyou will receive $80-$90 per evaluation, along with a mileage stipend and compensation for administrative duties.
