Job Description

This opportunity offers a rewarding role focused on delivering comprehensive primary and preventive care to senior living and home-bound patients. The objective is to provide in-place treatment when appropriate, helping avoid unnecessary hospitalizations and emergency room visits through regular health assessments. These assessments are conducted at least monthlyand more frequently if neededto monitor patients health status and ensure timely interventions.

Key Responsibilities:

  • Develop detailed problem lists and individualized medical treatment plans for each patient
  • Deliver primary, preventive, urgent, and acute care directly in home or senior living settings
  • Identify and stratify high-risk patients for increased clinical oversight and visits
  • Formulate contingency plans for patients with high-risk conditions
  • Collaborate consistently with patients primary care physicians to coordinate care
  • Provide hands-on bedside care, create care plans, develop medical orders, and work closely with facility staff and directors of nursing
  • Conduct single home visits for patients recently discharged from skilled nursing facilities as part of their transition plan

Qualifications:

  • Master of Science in Nursing (MSN) degree
  • Valid Oregon Nurse Practitioner (NP) license
  • DEA certification
  • Basic Life Support (BLS) certification
  • Experience in home health, hospice, or related settings preferred

Schedule:

  • Monday through Friday, 8 AM to 5 PM
  • No weekends or holidays
