Job Description
This role focuses on delivering comprehensive care to senior living and homebound patients, aiming to treat in place and reduce unnecessary hospitalizations. Monthly health assessments (or more frequent as needed) are conducted to provide timely care and resources that improve healthcare outcomes and prevent emergency room visits or hospital stays.
Schedule:
- Monday to Friday, 8 AM 5 PM
- No weekends or holidays
Key Responsibilities:
- Develop individualized problem lists and medical treatment plans for patients
- Provide primary, preventive, urgent, and acute care on-site
- Identify and closely monitor high-risk patients with more frequent clinical visits
- Create contingency plans for managing high-risk conditions
- Communicate and collaborate effectively with patients primary care physicians
- Deliver routine hands-on bedside care, develop care plans, and place orders within home or senior living settings
- Coordinate with facility staff, Directors of Nursing (DON), and primary care providers
- Conduct single visits for patients transitioning home from skilled nursing facilities (SNF) as part of discharge planning
Qualifications:
- Master of Science in Nursing (MSN) degree
- Valid Oregon Nurse Practitioner (NP) license
- Basic Life Support (BLS) certification
- DEA registration
- Experience in Home Health, Hospice, or related settings preferred
