Discover a rewarding per diem opportunity for experienced Nurse Practitioners interested in home health settings. In this role, you will perform comprehensive risk assessments for Medicare Advantage plan members, delivering care across Union County, including Blairsville, Suches, and Lake Nottely. Each assessment will generally take around 45 minutes and may involve either annual health evaluations or post-discharge visits. Scheduling for these visits is highly flexible, empowering you to tailor your shifts to best fit your lifestyle and personal commitments.
Schedule:
- Flexible shifts, Monday through Sunday
- Self-scheduling available
- Choose your preferred level of weekly or bi-weekly commitment
Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients medical history, current diagnoses, and medication use
- Educate patients and families about medical conditions and prescribed medications
- Identify gaps in care and offer actionable recommendations to resolve them
- No need for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Georgia
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience (preferred)
- MSN from an accredited nursing program
