Exciting opportunity for a dedicated Nurse Practitioner to deliver high-quality care through home health risk assessments in Bladen County, NC. This per diem role allows you to perform focused patient visits for Medicare Advantage plan members residing throughout Elizabethtown, Bladenboro, Bethel Townships, and surrounding areas. The position offers true flexibilityset your own schedule and commitment level while making an impact in your community.
Schedule:
- Flexible shifts available Monday through Sunday
- You choose your working days and hours
- Options for minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct comprehensive home health risk assessments (~45 minutes each) on assigned Medicare Advantage members
- Gather and evaluate medical history, current conditions, and medication profiles
- Provide clear education about medical issues, treatments, and medications
- Highlight any gaps in care, recommending solutions for improved outcomes
- No treatment planning or medication prescribing required
Preferred Experience & Qualifications:
- Active Nurse Practitioner license in the state of North Carolina
- Board Certification (AANP or ANCC)
- Master of Science in Nursing (MSN) from an accredited program
- Current BLS/CPR certification
- Minimum of six months experience as a Nurse Practitioner preferred
