Experienced Nurse Practitioners with a passion for home health are invited to deliver high-quality care through comprehensive risk assessments for Medicare Advantage members throughout Boone County, Missouri. This per diem position offers the flexibility to create your own schedule, with shifts available any day of the week to fit your lifestyle. Assignments are within Columbia, MO, and surrounding communities, ensuring local engagement and meaningful patient interactions.
Schedule:
- Flexible shifts available MondaySunday
- Self-scheduling for optimal work-life balance
- Minimum hourly commitment weekly or bi-weekly
Responsibilities:
- Complete in-home annual health risk assessments for Medicare Advantage plan members
- Gather and review medical, surgical, and medication histories
- Assess current and prior health conditions
- Provide education regarding health conditions and medications to patients
- Identify care gaps and provide recommendations, without prescribing or treatment plan creation
- Each assessment is expected to last approximately 45 minutes
Qualifications:
- Current Nurse Practitioner license in Missouri
- Active CPR certification (BLS)
- Board Certification through AANP or ANCC
- Masters degree in Nursing (MSN) from an accredited program
- At least 6 months of current Nurse Practitioner experience preferred
