Job Description
Deliver comprehensive care to senior living and homebound patients with a focus on improving health outcomes and reducing unnecessary hospital visits. This role emphasizes treating patients in place by conducting thorough health assessments at least monthly, and more frequently as needed, to provide timely care and resource support. The position is based in Gresham, OR, offering a structured Monday through Friday schedule, 8 AM to 5 PM, with no weekends or holidays required.
Key Responsibilities:
- Develop individualized problem lists and medical treatment plans for each patient.
- Provide primary, preventative, urgent, or acute care directly in patients homes or senior living communities.
- Stratify and identify high-risk patients for more frequent clinical visits.
- Create contingency plans tailored to manage high-risk conditions effectively.
- Collaborate closely with patients primary care physicians and facility staff to ensure coordinated care.
- Offer hands-on bedside care, develop care plans, write medical orders, and communicate with Directors of Nursing (DON) and other healthcare team members.
- Conduct single visits for patients transitioning home post-discharge from skilled nursing facilities (SNF).
Qualifications:
- Master of Science in Nursing (MSN) degree.
- Current Oregon Nurse Practitioner (NP) license.
- Basic Life Support (BLS) certification.
- DEA license.
- Previous experience in home health, hospice, or similar settings is preferred.
