As a Nurse Practitioner specializing in home health, you will conduct risk assessments for Medicare Advantage plan members residing in various areas of Adair County, including Kirksville, Greentop, and Novinger. Flexible scheduling is available, empowering you to choose shifts between Monday and Sunday and set your minimum weekly or bi-weekly commitment.
Schedule/Hours:
- Flexible shifts available Monday through Sunday
- Self-scheduled, create your own work hours
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in patient homes
- Review and document prior and current medical, surgical, and medication histories
- Provide patient education related to health conditions and prescribed medications
- Identify care gaps and offer recommendations to promote better health outcomes
- Visits can include annual assessments or post-discharge follow-ups as needed
- No treatment planning or medication prescribing required
Qualifications:
- Valid Nurse Practitioner license in Missouri
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- At least six months of nurse practitioner experience preferred
This position offers $80$90 per evaluation, along with a mileage stipend and compensation for administrative time.
