As a Nurse Practitioner in this vital role, you will use your clinical expertise to support people in their own homes across Alamance County, North Carolinaincluding Burlington, Graham, Mebane, and Elon. Each home visit typically lasts about 45 minutes and may involve either an annual health assessment or a post-discharge review.

Key Responsibilities
- Conduct comprehensive home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical histories, conditions, and medication use
- Offer education regarding medical conditions and medication adherence
- Identify gaps in care and make recommendations to enhance health outcomes
- Focus exclusively on assessmentno treatment plans or prescribing responsibilities
Qualifications
- Active Nurse Practitioner license for North Carolina
- Current BLS/CPR certification
- Board Certification via AANP or ANCC
- MSN from an accredited nursing program
- At least six months of nurse practitioner experience preferred
