Make an impact as a Nurse Practitioner specializing in home health risk assessments in Ashtabula County. This per diem opportunity allows you to conduct comprehensive health evaluations for Medicare Advantage members in their homes, offering flexibility and autonomy over your schedule.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Set your own hours
• Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Complete annual risk assessments during home visits with Medicare Advantage plan members
• Gather and evaluate patients medical history and medication profiles
• Educate patients about their health conditions and prescribed medications
• Detect and address gaps in care, making relevant recommendations
• Focus exclusively on assessmentsno treatment plans or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in Ohio
• Certification in CPR (BLS required)
• Board-certified through AANP or ANCC
• Minimum of 6 months recent NP experience preferred
• Master of Science in Nursing (MSN) from an accredited program
