Experienced nurse practitioners with a passion for home health are invited to join a dynamic, patient-focused team, conducting risk assessments to support Medicare Advantage plan members throughout Aroostook County, ME. This per diem opportunity empowers you to create a customized schedule while delivering high-value care in Fort Fairfield, Presque Isle, Caribou, Washburn, and surrounding communities.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual health risk assessments in members homes
- Evaluate medical history, active and prior conditions, and current medications
- Educate members and caregivers about management of chronic conditions and medication adherence
- Identify care gaps and communicate actionable recommendations
- Documentation and assessment onlyno treatment planning or prescribing necessary
Qualifications:
- Active Nurse Practitioner license in Maine
- Board Certification from AANP or ANCC
- Current BLS/CPR certification
- Minimum of 6 months recent NP experience preferred
- MSN degree from an accredited nursing program
