Job Description

Take on a rewarding position as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, you will serve the diverse communities of St. Landry County, including areas such as Opelousas, Eunice, and Port Barre. Your expertise will help enhance the health and wellbeing of Medicare Advantage plan members by conducting comprehensive assessments right in their homes.

Schedule:

  • Flexible shifts are available from Monday through Sunday
  • Create your own schedule for optimum work-life balance
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete thorough home health risk assessments, typically lasting around 45 minutes each
  • Assess patients prior and current medical history, conditions, and medication usage
  • Deliver patient and caregiver education regarding medical conditions and medications
  • Identify gaps in care and make clear recommendations for improved patient outcomes
  • Focus on evaluation and education; prescribing and treatment planning are not required

Qualifications:

  • Active Nurse Practitioner license in Louisiana
  • Current Basic Life Support (BLS/CPR) certification
  • Board certification through AANP or ANCC
  • At least 6 months of recent experience as a Nurse Practitioner preferred
  • MSN degree from an accredited nursing school
