Advance your career as a Nurse Practitioner specializing in home health services, conducting comprehensive risk assessments for Medicare Advantage members within Lane County, Oregon. This per diem opportunity features flexible scheduling, allowing you to create your own shifts anywhere from Monday through Sunday, with a minimum weekly or bi-weekly hourly commitment. Assignments may take you to various communities within the area, including Eugene, Springfield, Elmira, and Cottage Grove.
Key Responsibilities:
- Perform home health risk assessments, typically completed within 45 minutes per visit
- Review and document medical history, current health conditions, and medication usage
- Educate patients regarding their medical conditions and medications
- Identify care gaps and recommend appropriate next steps to improve patient outcomes
- No requirement to develop treatment plans or prescribe medications
Schedule:
- Flexible shifts available throughout the week
- Design your own work schedule to meet your personal and professional needs
- Minimum commitment required weekly or bi-weekly
Qualifications:
- Active Nurse Practitioner license in the State of Oregon
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current Basic Life Support (BLS/CPR) certification
- At least six months of experience as an NP is preferred
Compensation for this position is $80-$90 per completed evaluation, with an additional mileage stipend and paid administrative time.
