This per diem position offers flexibility and autonomy while enabling you to make a meaningful impact in the lives of individuals residing in Elizabethtown, Bladenboro, Bethel Township, and other local areas.

Schedule:

• Flexible shifts available seven days a week
• Self-scheduling to fit your availability
• Set a minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Complete comprehensive home health risk assessments during patient visits (approximately 45 minutes each)
• Review patients' medical histories, current conditions, and medication regimens
• Educate members about their health conditions and prescribed medications
• Identify and document care gaps, offering recommendations as needed
• Perform visits for a variety of appointment types, including annual health evaluations and post-discharge follow-ups
• Focus on assessment and education onlyno treatment planning or medication prescribing required

Qualifications:

• Active, unrestricted Nurse Practitioner license in North Carolina
• Master's degree in Nursing (MSN) from an accredited institution
• Six months or more of recent NP experience (preferred)
• Board certified by AANP or ANCC
• Current Basic Life Support (BLS/CPR) certification
