Provide essential care in the community by performing home health risk assessments with a flexible per diem opportunity focused on Medicare Advantage plan members. In this role, Nurse Practitioners deliver comprehensive assessments in El Dorado Springs and other Cedar County areas, including Stockton and Jerico Springs. Each visit, which typically lasts about 45 minutes, may be an annual health assessment or a post-discharge evaluation.

Schedule:

• Flexible shifts available Monday through Sunday
• Option to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform in-home annual risk assessments for Medicare Advantage members
• Review and document prior and current medical history and conditions, including medication usage
• Educate patients about their health conditions and prescribed medications
• Identify care gaps and recommend steps to address them
• Complete assessments without the requirement to create treatment plans or prescribe medication

Qualifications:

• Active Nurse Practitioner license in Missouri
• CPR certification (BLS) is current
• Board Certification required through AANP or ANCC
• At least 6 months of recent NP experience preferred
• MSN degree from an accredited nursing program
