Job Description

Experienced Nurse Practitioners are sought to conduct home health risk assessments for Medicare Advantage plan members within Baldwin County, GA. This per diem opportunity offers meaningful patient interactions in a supportive, flexible work environment, focusing on thorough annual and post-discharge evaluations in patients homes. Assessments typically last around 45 minutes, and visit assignments may include communities such as Milledgeville, Ivey, and Deepstep.

Schedule:

  • Flexible shifts available seven days a week
  • Opportunity to create your own work schedule
  • Minimum weekly or bi-weekly hourly commitment based on your availability

Key Responsibilities:

  • Complete comprehensive annual and post-discharge health risk assessments for Medicare Advantage plan participants
  • Review and document patients prior and current medical histories, conditions, and medication usage
  • Provide clear education on medical conditions and medication management
  • Identify any gaps in care and offer recommendations to support patient wellbeing
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Georgia
  • Board Certification by AANP or ANCC
  • Current BLS (CPR) certification
  • MSN degree from an accredited nursing program
  • At least 6 months of Nurse Practitioner experience preferred
