This position is ideal for Nurse Practitioners with a passion for preventive care and personalized patient interaction. Youll have the autonomy to create your own schedule and provide vital health evaluations across communities within Laurens County, GA.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum required commitment of hours weekly or bi-weekly
Responsibilities:
- Manage annual home health risk assessments for Medicare Advantage plan members
- Review and document prior/current medical history, conditions, and medication use
- Deliver patient education regarding conditions and medication management
- Identify potential gaps in care, offering recommendations for improvement
- Focus on assessment and educationno treatment planning or prescribing needed
Qualifications:
- Active Nurse Practitioner license in Georgia
- MSN degree from an accredited nursing program
- Current CPR (BLS) certification
- Board Certification (AANP or ANCC) required
- At least 6 months of recent Nurse Practitioner experience preferred
As part of this specialized per diem team, you will earn $80$90 per evaluation, plus a mileage stipend and compensation for administrative time. This role lets you balance your professional expertise with your preferred level of commitmentideal for those who value flexibility and independence.
